What to expect during your telehealth SAP evaluation

Every DOT SAP evaluation on this site is conducted by secure telehealth — no travel, no waiting rooms. Here's exactly what happens, from clicking the login link to the follow-up summary sent to your employer.

  1. 1. Schedule your appointment

    Call 717-614-0299 or use the online form. We confirm your DOT mode (FMCSA, FAA, FTA, FRA, PHMSA, USCG), your employer's DER contact, and pick a time — usually within 2–3 business days.

  2. 2. Receive your intake packet

    You get an email with a short intake form (about 10 minutes), a HIPAA/consent form to e-sign, and your secure telehealth link. Complete everything at least 24 hours before your appointment.

  3. 3. Set up your space

    Find a private, quiet room. Test your camera and microphone using the browser link we provide. Have a photo ID and any employer/MRO paperwork within reach.

  4. 4. Log in and join the video room

    Ten minutes before your appointment, click the link in your confirmation email. No app, no account, no download — the room opens in your browser. Your SAP admits you when the session begins.

  5. 5. Identity verification and consent review

    Your SAP verifies your photo ID on camera and reviews the consent, confidentiality, and DOT reporting requirements. This takes about 5 minutes.

  6. 6. The clinical assessment (60–90 minutes)

    A conversational, DOT-structured interview covering the testing event, your history, current circumstances, and any prior treatment. Not an interrogation — candid answers lead to an accurate recommendation and the fastest path back to duty.

  7. 7. Recommendation delivered on the same call

    Before you log off, your SAP tells you the specific education or treatment recommendation and what completion will look like. You'll know exactly what's next before the session ends.

  8. 8. Written report sent to your DER

    Within one business day, your SAP sends the written recommendation to your employer's Designated Employer Representative. Only the recommendation and your compliance status are shared — never your clinical history.

  9. 9. Follow-up summary after program completion

    Once you complete the recommended program, you return for a follow-up telehealth session. Your SAP confirms compliance, clears you for the return-to-duty test, and issues your follow-up testing plan to the DER.

What device do I need for a telehealth SAP evaluation?

Any smartphone, tablet, laptop, or desktop with a working camera, microphone, and a stable internet connection. We recommend Chrome, Safari, or Edge — no app download or account creation required. You'll click a secure link in your email and join the video room directly in your browser.

Is the telehealth session HIPAA-compliant?

Yes. The video platform is HIPAA-aware, end-to-end encrypted, and nothing is recorded. Only you and your SAP are in the session.

What if my connection drops mid-session?

Just click the same link again to rejoin. If the drop is longer than a few minutes, we'll switch to a phone backup and continue where we left off. You will not be charged extra and you will not have to restart the evaluation.

Can I do the follow-up evaluation by telehealth too?

Yes. The initial evaluation, the follow-up evaluation, and any check-ins in between can all be conducted by secure telehealth from anywhere in Pennsylvania.

Where should I be during the call?

A private, quiet room where no one else can overhear. Not a car, a break room, or a public place. Federal privacy rules require a confidential setting for the clinical assessment.
